Field Documentation Officer | Community Kidney Health Project • Field-Based | Project-Based | As-Needed Deployment

Job Overview:

We are looking for dependable, organized, and detail-oriented individuals to join our team as Field Documentation Officers for community and healthcare-based projects.

In this role, you will serve as the onsite field representative of the organization and provide operational support to the Project Manager during medical missions, community activities, and healthcare deployments. The Field Documentation Officer plays a key role in ensuring that field operations, documentation processes, and project reporting are conducted accurately and efficiently.

Responsibilities include assisting with onsite coordination, organizing and reviewing project forms, validating and encoding field data, monitoring daily field activities, and submitting operational updates and reports to the Project Manager. The role also involves coordinating with healthcare personnel, local government units (LGUs), partner organizations, and community stakeholders to support smooth project implementation.

Projects may be conducted in:

  • Barangay medical missions and community health caravans
  • City or Provincial Health Offices (CHOs/PHOs)
  • Rural Health Units (RHUs)
  • Barangay health centers, clinics, and hospitals

This role is ideal for individuals who are adaptable, highly organized, detail-oriented, and comfortable working in fast-paced field environments involving healthcare operations and community engagement.

 

Deployment Areas

Current project deployments are primarily located in:

  • Quezon City
  • Antipolo City
  • Taytay, Rizal
  • Makati City (office-based assignments)

Qualified applicants may also be considered for future projects and deployments in other cities and municipalities as operations expand.

Duties & Responsibilities:

Documentation and Data Validation

  • Collect, organize, and review physical forms and field documents
  • Validate encoded data against submitted physical forms for completeness and accuracy
  • Identify, document, and help resolve data discrepancies or missing information
  • Assist in organizing and maintaining project documentation and records
  • Prepare and submit daily or weekly documentation and operational reports to the Project Manager

 

Field Coordination and Operations

  • Assist during medical missions, community activities, and project deployments
  • Coordinate with encoders, event organizers, healthcare personnel, LGUs, and partner organizations as needed
  • Support onsite operations and help ensure smooth field implementation
  • Maintain close communication with the Project Manager regarding field updates, concerns, and operational issues
  • Monitor and document attendance of encoders and field personnel
  • Assist in documenting field expenses, reimbursements, allowances, and operational logistics
  • Coordinate schedules, deployment updates, and field-related communications as needed

 

Qualifications & Requirements:

  • Bachelor’s degree in Public Health, Community Development, Healthcare Administration, Social Sciences, Business Administration, or related fields
  • Experience in field operations, community engagement, project coordination, healthcare projects, or NGO-related work is an advantage
  • Open to fresh graduates with strong leadership and organizational experience
  • Willing to travel and conduct fieldwork in communities and partner health facilities
  • Comfortable working in fast-paced and operationally dynamic environments
  • Must be highly organized, dependable, and proactive
  • Experience coordinating with LGUs or healthcare stakeholders is an advantage
