Participation & Data Privacy
- Voluntary: Joining is completely your choice. You may skip optional questions or stop before submitting without affecting your healthcare services.
- Data Collection: We collect email addresses via Google Forms strictly to verify response validity and prevent duplicate submissions. Emails are never used for advertising.
- Confidentiality: Please do not include personal identifiers (names, phone numbers) in open responses. Only authorized 101 Health Research staff can view individual responses. DOH-MMCHD receives only aggregated summary reports.
- Data Retention: Secured for five (5) years and deleted in compliance with the study’s data management plan and the Data Privacy Act of 2012.
